We are an ECM provider partnering with Medi‑Cal managed care plans across California and we follow Medi‑Cal program rules, contract requirements, and applicable privacy and civil‑rights laws. No remuneration for referrals.

All ECM program descriptions on this site align with DHCS ECM guidance; final eligibility and services are determined by the member’s Medi‑Cal managed care plan and its contracted ECM providers.

We collect only the minimum information needed to respond to your request—typically name, phone, email, preferred language, city/ZIP, and your message; please do not include medical details. We do not sell personal information and we do not share personal information for cross-context behavioral advertising. We may disclose information to service providers/contractors (e.g., Medi-Cal managed care plans and contracted community-based organizations) solely to deliver ECM services. We retain contact information up to 24 months and security logs up to 180 days. We honor Global Privacy Control (GPC) signals. See our Privacy Policy for details and your rights.

As a provider of ECM services, we are required by federal law to maintain the privacy of your Protected Health Information (PHI). We will only share your information with your health plan, your doctors, and authorized community partners involved in your care plan.

Enhanced Care Management (ECM) is a free, voluntary benefit provided through Medi-Cal managed care plans (MCPs) that adds coordination without replacing existing doctors or benefits. Eligibility depends on your specific plan; members with a monthly Share of Cost or those in long-term care are typically ineligible. We are a plan-neutral ECM provider contracted with MCPs and receive no remuneration for referrals. We strictly follow DHCS Referral Standards by acknowledging, verifying, outreaching, and assigning a Lead Care Manager.
